Target Case Manager Position Description

ESSENTIAL RESPONSIBILITIES:
Implementation of the Wraparound Model
Coordination of Child and Family Teams
Linkage to community resources 
Meeting with the families face to face in the community i.e. court, schools, psychiatric hospitals, homes etc. for the purpose of coordinating services
Advocate for the family’s needs in the community while working with other community professionals.
Help empower families to become self-sufficient
Attend regular individual/group supervision and team meetings.
Maintain client files and program records, court reports, and develop reports as required.
Comply with HIPAA and Data Privacy Practices.
Positively represent agency, program and participant needs.
This position will require the ability to work outside of regular business hours, evenings and some weekends.
Maintain established standards for direct client contact.
Maintain client confidentiality according to established policies, federal and state statutes, as evidenced by obtaining needed releases of information and maintaining updated releases of information.
Provide complete documentation for services rendered within established time frames, including daily log, progress notes, assessments and case plans, and other required documentation.
Complete assigned paperwork in compliance with standards, including time sheets, mileage logs, leave requests, clients’ records, client appointments and management information system.
Other duties as assigned.


QUALIFICATIONS: 
Required: 
-Bachelors/Masters of Arts or Science Major: Social Work, Psychology, Counseling, Education, Criminal Justice, Business, Special/Health Education, Marriage and Family Therapy, Mental Health Counseling, or related human services field.
-Wraparound Training completed
-History of providing Targeted Case Management services.
Preferred:
-Certified Behavioral Health Case Manager 
-A minimum of 1 year of experience working with children
-Wraparound Certified
-SOAR Certified

SPECIAL KNOWLEDGE AND SKILLS:
Must possess strong oral and written communication skills.
Knowledge of public speaking required.
Must be highly motivated and self-directed.
Knowledgeable of community resources, service referrals and linkage.
Computer literate
Great time management and organizational skills 
Bilingual: Spanish, Creole, French, Portuguese
